A child puts down their spoon at lunch and says they're full. Twenty minutes later, they want to eat something again. Were they really full at lunch, and why do they want more food so soon?

Short answer: a child is born with biological mechanisms for detecting hunger and satiety, but some children recognize and follow these signals more easily than others. This is influenced by their innate characteristics, previous experiences with food, and the environment in which they eat.
Trusting a child's appetite therefore doesn't mean leaving all feeding decisions entirely to them. Adults provide regular meals, diverse and nutritionally adequate offerings, and a calm environment without rushing or forcing. The child then decides whether to eat and how much to eat from what's offered.

How does a child sense they've eaten enough?

During a meal, a child doesn't calculate how much food or energy they've consumed. As food enters the stomach, it fills and stretches, and information about its fullness travels via nerves to the brain. During digestion, hormones are also released that signal to the brain that the body is receiving nutrients and energy. Hunger therefore gradually subsides. These signals contribute to the child stopping eating during a meal and maintaining a feeling of satiety afterward [1].

However, during feeding, the brain doesn't only receive messages from the digestive system. The child simultaneously responds to the taste and appearance of food, the pace of eating, their mood, and what's happening at the table. How much they eat is therefore not solely dependent on their hunger and satiety.

An important external guide is also the size of the portion offered. If a child receives a large portion of pasta, they will often eat more than if they receive a smaller initial portion [2]. However, in some children, too large a portion can have the opposite effect: it puts them off before they even start eating. A smaller initial portion with the possibility of seconds therefore allows the child to more easily adjust the amount consumed to their appetite.

Does a child always eat exactly as much as they need?

Not entirely. The ability to regulate the amount consumed is typically most pronounced in early life. An infant doesn't know how many milliliters of milk they need, but they slow down feeding, release the breast or bottle, or turn their head away when they've had enough. With growing up, the attractiveness of food, the feeding environment, and adults' responses increasingly influence their eating. Some aspects of appetite regulation can therefore gradually weaken, even though the child is otherwise becoming better at controlling their behavior [3].

This is also evident in caloric compensation studies. After a more energy-rich snack, children on average ate less at the next meal, and after a less energy-rich snack, they ate more. They therefore adjusted the amount of food to some extent based on how much energy they consumed earlier, but some did so much more successfully than others. Poor caloric compensation can contribute to a child consuming more energy than they need, especially in an environment where large portions and energy-rich food are frequently available [1, 3].

So what does the desire for more food from the introduction mean? The child may have genuinely been full at lunch, but then the desire to eat reappeared at the sight of other food or even at the thought of it. The desire for food can appear even before physical hunger returns, as we don't eat only because of the need for energy, but also because of the anticipated pleasure from food. Adults know this too: after lunch we can be completely full, but at the sight of our favorite dessert, there's often still a little room [3].

Responsive feeding: the child decides the quantity, the adult provides the offering

The question of how much a child will eat must be separated from the question of what we will offer them. When deciding on quantity, we let them follow their appetite, but we cannot yet leave the choice of food entirely to them, as they don't have the knowledge and experience needed for a diverse and nutritionally adequate diet.

If they don't touch a particular dish, this isn't necessarily a sign of satiety. The dish may be unfamiliar to them, they may be bothered by its taste or texture, or they may have had an unpleasant experience with it [1]. The adult therefore doesn't have to choose between forcing and completely removing the rejected food from the child's menu. They can accept the child's refusal and offer the food again on another occasion without persuading, conditioning, or rewarding [4].

In responsive feeding, adults don't conclude whether the child has eaten enough based solely on the amount of food left on the plate. They also observe their behavior. A child can show satiety by slowing down eating, putting down utensils, turning their head, or saying they've had enough. We understand these signs as information about their current appetite, not as behavior we need to correct. Responsive feeding is therefore not passively leaving decisions to the child, but attentively guiding the meal without controlling every bite [4].

What to do when a child eats little or wants an additional portion?

Responsive feeding is most challenging when a child eats considerably less or more than we expected. An almost full plate quickly raises concerns that they've eaten too little, while a request for seconds raises the question of whether there wasn't already enough. In the first case, we start encouraging them to take a few more bites, and in the second, we'd prefer to decide that they've eaten enough. In both cases, we judge their hunger or satiety instead of them.

If either of these situations frequently recurs, we first check what initial portions we're offering the child. The Guidelines for Nutrition in Educational Institutions, which contain indicative food quantities for different age groups, can help us [5]. They help us estimate how much food to offer, but they don't determine how much an individual child must eat.

One almost untouched meal doesn't yet mean a child is consuming too little food. But if they eat very little every day, we check whether they frequently snack between meals, fill up on milk or juice, or meals are too close together. If their growth follows a normal pattern, they feel well, and eat sufficiently diversely, there's no need to force them to eat more. If there's weight loss, growth stagnation, or persistent feeding difficulties, we consult with a pediatrician.

If after the first portion a child says they're still hungry, we offer them a smaller addition composed of different parts of the meal. If they want to eat only pasta, we don't force them to also consume vegetables, but we don't keep piling on pasta again and again. After eating the offered amount, they can choose from other food that's still available, or end the meal. The adult thus sets boundaries on the offering, but doesn't decide instead of the child what they must eat from what's offered.

Even when a child frequently wants more food, this doesn't yet mean they're eating too much. Greater appetite can accompany a period of faster growth or greater physical activity, or the initial portions may be too small. It's also sensible to check how quickly the child eats, as fast eating can make it harder to detect satiety in time [1]. However, if the child's usual growth pattern also changes noticeably with increased appetite, we consult with a pediatrician.

Food should not become a reward or means of pressure

Statements like "just three more spoonfuls," "you haven't eaten anything," or "if you eat the vegetables, you'll get dessert" are usually said out of concern. With them, however, the adult takes over deciding how much the child must eat, even when the child signals they've had enough. The child may then eat a few more bites, not because they're still hungry, but because they want to please the adult or earn the promised dessert. Adult pressure can thus override the child's signs of satiety [4].

The promise of dessert can have another effect. The problem isn't the dessert itself, but the role it's given. If dessert becomes payment for a consumed dish or an empty plate, the less popular dish can become even less attractive, and the dessert something especially valuable. Using food for rewards has also been linked in studies to later emotional eating in children [6].

Dessert should therefore remain dessert, not payment for consuming an unpopular dish. If it's planned as part of the meal, we don't condition it on the number of bites or an empty plate.

So, does a child really know how much they need to eat?

Not always and not in every situation. A child is still learning to distinguish between hunger, satiety, and the desire for food, and their appetite isn't an infallible measure of current needs. But an adult also cannot know exactly how much a child must eat at a particular meal. We best support the development of this ability by providing an appropriate offering and meal rhythm, without taking control over every bite.
